7 RTSM Providers for Clinical Trials in 2026
A randomization and trial supply management (RTSM) provider handles two of the most operationally sensitive parts of a clinical trial: assigning patients to a treatment arm according to protocol, and making sure the right investigational drug supply reaches the right site before that patient's visit. This kind of RTSM provider has historically been described using terms like IVRS, IWRS, and IRT, and while the underlying technology has evolved from phone-based systems into configurable cloud platforms, the core job — protecting randomization integrity while keeping supply chains stocked and dosing on schedule — has stayed the same.

Founded in October 2015 and based in Wellesley, Massachusetts, the company has since added companion products including 4C Supply for broader forecasting and scenario modeling, and UltimaRand for structured randomization and kit list generation on more focused studies. 4G Clinical maintains facilities and staff across the US and Europe, offering advisory services alongside its core RTSM technology. The company positions its supply forecasting capability, built on statistical models rather than manually managed algorithm parameters, as a key differentiator from other RTSM systems.
Atreo
Atreo builds its RTSM platform around rapid deployment, stating it can complete a system build from kickoff to launch in one to two weeks and implement mid-study changes at no additional cost. Founded in San Francisco in 2021, the company entered a 2025 partnership with N-SIDE, a clinical trial supply chain analytics firm, to integrate Atreo's RTSM platform with N-SIDE's forecasting tools. Atreo has deployed studies across all trial phases in more than 50 countries, and in 2025 released a version 2.0 update to its platform developed in collaboration with large pharmaceutical and biotech clients. The company's team includes RTSM specialists who previously worked at larger, more established RTSM providers, including Endpoint Clinical.
Endpoint Clinical
Endpoint Clinical runs its RTSM business on PULSE, a proprietary interactive response technology platform built around pre-validated, configurable study components for patient randomization, site management, and drug supply management. Founded in 2009 and headquartered in Raleigh, North Carolina, the company was acquired by Arsenal Capital Partners in 2024 and now operates from offices across the US, Europe, and Asia with more than 600 employees globally. In December 2025, Endpoint Clinical was named a Leader in the Everest Group's Life Sciences RTSM Products PEAK Matrix Assessment, which evaluates providers on market impact and technical capability. The company also offers DRIVE, a separate clinical supplies management tool, alongside its core PULSE platform.
Suvoda
Suvoda specializes in RTSM for complex, life-sustaining studies, with roughly 65 percent of the more than 6,000 trials it has supported concentrated in oncology, central nervous system, and rare disease. Founded in 2013 and headquartered outside Philadelphia in Conshohocken, Pennsylvania, the company completed a merger with Greenphire, a clinical trial payments and patient support firm, in April 2025, combining RTSM with financial and patient support tools under one ownership structure. Suvoda's platform includes an embedded AI assistant called Sofia, which the company says can cut the time from study kickoff to user acceptance testing by up to 80 percent. The company states that 34 percent of trials that earned FDA approval since 2024 ran on its platform.
5. YPrime
YPrime markets its IRT platform on speed metrics, citing 50 percent faster study start-up times and four-times-faster system changes for protocol amendments compared with the industry baseline it references in its own materials. Based in Austin, Texas and founded in 2006, the company also offers eCOA and eConsent as part of a unified clinical trial technology platform alongside its core randomization and supply management product. In one published case study, YPrime's IRT supported an adaptive Phase 1 oncology trial with complex stratification requirements, completing study setup in under four weeks. The company backs its technology with a 24/7 global help desk staffed by eClinical experts familiar with a given protocol.
IVR Clinical Concepts
IVR Clinical Concepts (IVCC) has focused specifically on Interactive Voice and Interactive Web Response applications since its founding in 2007 by John Hudak, predating the industry's more recent shift in terminology from IVR/IWR toward RTSM and IRT. Based in Saratoga Springs, New York, the company builds electronic patient diaries and telephone scripts customized to each client's study, alongside its core randomization and drug supply and inventory management applications. IVCC's systems are designed to interface with electronic data capture and other digital platforms, supporting real-time reporting across the phases of a clinical study from patient screening through drug supply management. The company has remained privately held and independent throughout its operating history, without external funding rounds.
Bluefin Clinical
Bluefin Clinical focuses specifically on clinical supply forecasting, letting users calculate participant and supply demand from simple parameters like enrollment rates and dates rather than building a full randomization system from scratch. Founded in 2023 by CEO Andy Maltun, the company's first product, Bluefin Forecasting, addresses what it describes as a persistent gap in demand and supply forecasting accuracy across the industry. Bluefin's platform automatically populates live actuals as a study progresses, narrowing forecasting uncertainty without requiring users to manually import or reconcile datasets from separate systems. The company has stated a broader roadmap toward an end-to-end clinical supply platform, with plans to add functionality including RTSM alongside its current forecasting-focused product.
