12 Decentralized Trial (DCT) Companies for Clinical Trials in 2026

A decentralized clinical trial moves some or all of a study's activities away from a traditional research site and into a patient's home or community, using a mix of software platforms, wearable devices, and mobile healthcare staff to keep data flowing without requiring every visit in person. A DCT provider might specialize in the eCOA and eConsent software that captures patient-reported data remotely, the mobile nursing and phlebotomy services that bring a physical visit to a patient's door, or the patient logistics support that gets participants to a site when a visit can't be avoided. For Sponsors and CROs evaluating decentralized trials providers, the fit often depends on how much of a study needs to be virtual versus how much just needs the burden of an in-person visit reduced.

Medable provides a cloud-based platform supporting eConsent, telemedicine, eCOA, and remote patient monitoring for decentralized and hybrid clinical trials. Co-founded in 2012 by Dr. Michelle Longmire, the Palo Alto-based company states its SaaS platform has been deployed in more than 400 decentralized and hybrid trials across 70 countries, and it launched an agentic AI platform in 2025 to help clinical research associates manage study workflows. Medable reported roughly 80% revenue growth in 2024 as eCOA adoption broadened across its customer base.

Signant Health

Signant Health provides a unified eClinical platform combining EDC, eCOA, eConsent, IRT, and clinical supplies management, formed in 2018 when private equity firm Genstar Capital merged CRF Health and Bracket into CRF Bracket before relaunching under the Signant name in 2019. The company states its platform has supported more than 3,000 clinical trials over more than 30 years, and it grew its decentralized trial capabilities through the 2020 acquisition of VirTrial and the 2023 acquisition of DSG. Signant Health is headquartered in Blue Bell, Pennsylvania.

Clario

Clario provides eCOA, cardiac safety, respiratory, and medical imaging endpoint technology for clinical trials, formed in 2021 through the merger of eResearchTechnology (ERT) and Bioclinica under Nordic Capital and Astorg ownership. The company traces its cardiac safety roots to 1972 and states it has supported more than 30,000 clinical trials and contributed to more than 700 FDA and EMA drug approvals. In October 2025, Thermo Fisher Scientific announced a definitive agreement to acquire Clario for approximately $8.9 billion, a deal expected to bring Clario's endpoint technology under Thermo Fisher's ownership.

Clinical Ink

Clinical Ink provides an eSource platform that integrates Direct Data Capture, eCOA, ePRO, and eConsent into a single clinical data system, positioning itself as an alternative to EDC systems that still rely on paper source documents being transcribed and later verified. Founded in 2007 and based in the United States, the company states its SmartSignals-style eSource approach removes the tethered, siloed systems that traditional data capture depends on, supporting fully remote, virtual, and hybrid trial designs. Clinical Ink's platform also supports neurocognitive testing and digital biomarker capture.

Huma

Huma operates a remote patient monitoring and digital clinical trials platform, evolving from a 2011 patient monitoring startup founded in London into what the company now calls the Huma Operating System, a broader infrastructure layer for AI-driven healthcare and research software. The company states its technology has been used by more than 3,000 hospitals and clinics and 1.8 million active users across more than 70 countries, and its platform supported all-Covid-19 remote patient monitoring for the NHS during the pandemic. Huma raised an $80 million Series D round in 2024 to launch its Huma Cloud Platform for external customers.

uMotif

uMotif provides a cloud-based eCOA and ePRO platform designed around patient engagement, supporting studies from Phase I-III through fully decentralized and real-world research with a patented data-capture interface the company calls 'motif.' Based in London and hosted across the US, Europe, and China, the company states its platform is used in more than 30 countries and complies with GCP, 21 CFR Part 11, and GDPR requirements. uMotif raised $25.5 million in growth funding and formed a strategic partnership with Syneos Health in 2023 to build an integrated patient-centric eClinical platform.

Aparito

Aparito developed the Atom5 platform, which combines mobile apps, video assessments, and wearable devices to capture patient-generated data for rare disease and other decentralized clinical trials. Founded in 2014 by Dr. Elin Haf Davies and headquartered in Wrexham, Wales, the company states its platform is available in 193 countries and 125 languages. Eli Lilly and Company acquired Aparito in July 2024, and the company continues to operate from its Wrexham base as a wholly owned Lilly subsidiary.

EmVenio Clinical Research

EmVenio Clinical Research, formerly known as PCM Trials, recruits, trains, and manages Certified Mobile Research Nurses who conduct clinical trial visits, including phlebotomy, directly in patients' homes to support decentralized and hybrid study designs. The company rebranded from PCM Trials to EmVenio Clinical Research in August 2025 as a division of Professional Case Management, carrying forward its mobile nursing model under the new name. EmVenio's nurses typically hold at least two years of related clinical experience, often from emergency department or ICU backgrounds.

Medical Research Network (MRN)

Medical Research Network provides home-based clinical trial visits and site nursing support, describing itself as the longest-standing organization in this niche since its founding in the UK in 2006. The company states its Home Trial Support service has delivered in-home visits for more than 230 trials, and it operates a global Site Network of community-based research locations that expanded through its 2024 acquisition of VCTC. MRN is headquartered in Milton Keynes, England, with additional offices across North America, Europe, and Asia.

Elligo Health Research

Elligo Health Research brings clinical trials directly into physician practices through its PatientSelect and SiteSelect models, using electronic health record data and its proprietary IntElligo technology to identify and prescreen protocol-eligible patients within existing patient-physician relationships. Founded in 2016 and headquartered in Austin, Texas, the company states its network spans more than 115 hospitals and health systems, 200 healthcare-based sites, and 100 research-based sites. Elligo positions itself as an integrated research organization rather than a traditional CRO or site network.

mdgroup

mdgroup provides patient travel, accommodation, and logistics support for clinical trial participants, along with decentralized clinical services that bring nursing visits and equipment directly to patients. Founded in 2002, the company states it maintains a 24/7 global team of patient navigators and reports a 99% patient satisfaction rate across its travel, reimbursement, and in-home coordination services. mdgroup's clinical logistics division separately manages the global delivery of equipment needed to support decentralized and hybrid trial visits.

Current Health

Current Health provides a remote patient monitoring platform built around an FDA-cleared wearable biosensor, clinical dashboards, and telehealth integration, used by health systems for hospital-at-home programs and by pharmaceutical sponsors for decentralized trial data collection. Founded in 2014 in Edinburgh and Boston, the company was acquired by Best Buy Health for roughly $400 million in 2021, then divested and reacquired by co-founder Christopher McGhee in June 2025 as an independent company. Current Health states its platform has supported more than 70,000 patients and partners with several of the world's largest pharmaceutical companies on clinical research.
